Emergency Consent Form

A $50.00 nonrefundable registration fee is required for each student upon registering.


Due by June 1st is one month’s tuition which will be applied to the last month of the school year. Failure to pay the one month’s tuition by June 1st may result in losing your child’s reserved space in the classroom.  (September’s tuition is due at the start of school in September.)


After registration you will receive 3 forms by mail that have to be completed and returned before the start of school:

1.  Child’s Medical Report (completed by child’s pediatrician)

2.  Child’s Information Record

3.  Emergency Consent form


*We realize some medical forms might be delayed due to pending appointments and should be brought in ASAP.

Children enrolled in the PEANUTS NURSERY SCHOOL Program are registered for the entire school year.  Tuition is computed at an annual rate which includes only the actual days of school.  That amount is then divided by 10 months to arrive at a monthly tuition.

     Tuition is as follows:                   2019/2020

       2 Days (Tues./Thurs.)                $195 per month

       3 Days (Mon./Wed./Fri.)             $220 per month

       5 Days (Mon.-Fri.)                      $320 per month

All tuition payments are due by the 1st of the month.  A $5.00 late fee will be assessed after the 10th of the month.  There will be a $15.00 fee for all returned checks.
